KINDERGARTEN AND TRANSFER STUDENT REGISTRATION

Entering any new school can be a challenging time for children; in the Yough School District our doors are always open to help assist you in any way possible. If you have any questions or concerns, please feel free to contact your building administration at any time. Your role in the learning experience is critical and your support is necessary for us to provide your child with a world-class educational opportunity.
K REGISTRATION/READINESS SCREENING
Kindergarten registration is under way! You may pick up the registration packet for Kindergarten at your child's elementary school between 10 am and 3 pm M-F school days by taking the following required documents:
- 2 proofs of your residency in the district (e.g., utility bill, driver's license, morgage or lease)
- Your child's birth certificate
- Your child's immunization records

Upon presenting these documents you will receive our registration packet to take home and complete. Please return the completed registration forms to the school by April 23.
Upon receipt of the completed packet, the school staff will give you an appointment for the Kindergarten screening in May. See the K registration flyer 2021-22.pdf for more info.
TRANSFER STUDENTS ENROLLMENT
Registration forms are available for the 2020-2021 and 2021-2022 school years for transfer students.
If you are not able to print the forms, please pick a packet up at the District Central Administration Office at the HS building, Door 4 or contact us and provide your name, address, children's names, grades and anticipated date of enrollment. We will mail you a registration packet. If you do have print capabilities the forms can be accessed in the adjacent block for download. Please complete and print the forms.
You will also need 2 proofs of residency, the birth certificate, and immunization records when submitting your registration packet. If any custody documents are in place, they must be presented at the time of registration. Please see the right-hand section for directions on completing registration.
PROOF OF RESIDENCY NOTES: Utility bills, tax records, settlement agreement, and rental lease agreement are examples of acceptable proof of your residency in the Yough School District. If your family is living with another district resident who owns or rents the home and therefore you do not have such proof of residency for your family, please ask the homeowner to complete the following form (requires notarization) and provide 2 proofs of their residency.
GUARDIANSHIP BY DISTRICT RESIDENT NOTES: If a parent or legal guardian grants guardianship to a district resident, who will be responsible for the education of the child for the school year, the following forms must be completed:

TRANSFER STUDENT REGISTRATION SUBMISSION
You may pick up the registration packet at the district central administration office, Door 4 at the High School or print from this webpage.
IMPORTANT Completion of registration by appointment only
Due to building entry limitations and precautions at this time, you must make an appointment with the district registrar to present all of the required documents and completed forms (it will not be necessary to bring your child to this appointment). Please limit the number of family members at the appointment to 1 or 2 if possible.

Upon arrival to the school, the following procedures must be followed by each person:
- Wear a mask
- Complete a COVID-19 sign in sheet
- Have temperature screened. Entry into appointment for temperatures 100.3 and lower (100.4 or higher is considered fever by CDC guidelines)
- Please maintain social distancing of 6 feet apart from other people
- Masks required upon entry and throughout the time while in the building.
